ABSTRACT

Juvenile idiopathic arthritis(JIA)is a common chronic rheumatic disease in childhood.It is not a single disease, but a group of heterogeneous diseases including different subtypes.The etiology and pathogenesis of JIA are still unclear.It is currently believed that immune response disorders play an important role in its pathogenesis and development.Different subgroups of adaptive immune cells(including T lymphocytes and B lymphocytes, etc.)may participate in the pathogenesis of different subtypes of JIA, leading to different clinical manifestations.However, the specific mechanism of action and related molecular signaling pathways have not been fully elucidated.This paper reviews the latest research results in recent years and explores the role of different types of adaptive immune cells in the development of various subtypes of JIA, which will help the precise diagnosis and individualized treatment of each subtype of JIA.

ABSTRACT

Objective@#To analyze the curative effect and nursing care of MAP complicated with paralytic intestinal obstruction by modified retention enema and traditional retention enema.@*Methods@#Select Queshan People′s Hospital from May 2013 to May 2018 treated 250 cases of acute pancreatitis patients with paralytic ileus, according to random number table method to all of the patients were randomly divided into two groups, control group and test group two groups of patients were given conventional treatment nursing of acute pancreatitis, which on the basis of the control group patients give traditional retention enema treatment, while the experimental group patients give improved retention enema surgery treatment; Total enema times, liquid retention time and remission time of PI symptoms after enema were recorded for the two groups, and laboratory indicators such as comfort level, C-reactive protein level and serum creatine phosphokinase were compared between the two groups.t@*Results@#The comfort rate of the experimental group was 84.00% (105/125), and that of the control group was 61.60% (77/125). The difference between the two groups was statistically significant (χ2=13.034, P<0.05). After treatment, the total enema frequency, drug retention time, and PI symptom relief time were (4.10±1.23) times, (25.39±4.28) min, (2.53±0.81) d, and the control group were (6.29±2.19) times, (8.72±2.11) min, (3.92±1.23) d, the difference between the two groups was statistically significant (t=5.203, 9.349, t=5.293, both P<0.05). The levels of creatine phosphokinase in the experimental group were (87.39±76.20), (103.24±75.38), (70.92±64.22) U/L after 1, 3, and 5 days of hospitalization, and the control group was (106.28±81.29), respectively. 213.29±73.29), (175.28±58.99) U/L, the difference between the two groups was statistically significant (t=19.276, 24.334, 25.394, both P<0.05). There was no significant difference in the C-reactive protein level between the experimental group and the control group (P>0.05). The C-reactive protein levels were (11.72±2.36) and (9.52±2.17) after 3 days of hospitalization and 5 days after hospitalization, the control group were (14.02±2.37), (11.08±2.76), and the difference between the two groups was statistically significant (t=15.342, 13.029, both P<0.05).@*Conclusion@#Compar enema, the modified retention enema can effectively reduce the total enema times of the patients, prolong the retention time of the patients' liquid while maintaining the temperature, and greatly shorten the remission time of the patients'PI symptoms. It is worth popularizing in clinic to improve the patients' comfort, C-reactive protein, serum creatine phosphokinase and other laboratory indexes.

There was no significant difference in the C_reactive protein level between the experimental group and the control group (P>0.05). The C_reactive protein levels were (11.72 ± 2.36) and (9.52 ± 2.17) after 3 days of hospitalization and 5 days after hospitalization, the control group were (14.02±2.37), (11.08±2.76), and the difference between the two groups was statistically significant (t=15.342, 13.029, both P<0.05). Conclusion Compar enema, the modified retention enema can effectively reduce the total enema times of the patients, prolong the retention time of the patients' liquid while maintaining the temperature, and greatly shorten the remission time of the patients'PI symptoms. It is worth popularizing in clinic to improve the patients' comfort, C_reactive protein, serum creatine phosphokinase and other laboratory indexes.

ABSTRACT

Objective To explore the association of resting heart rate (RHR) and coronary artery lesions with left ventricular function in coronary heart disease for secondary prevention of this disorder.Methods We collected data on 239 patients with coronary heart disease confirmed by coronary angiography or coronary CTA examination in Military General Hospital of Guangzhou Command from January 2016 to August 2017.The clinical data and results of selective coronary angiography were studied.According to baseline RHR,the patients were divided into 4 groups with RHR < 60 min (group A,32 cases),60 to 69 min (group B,60 cases),70 to 79 min (group C,68 cases),and ≥ 80 min (group D,79 cases) for analyzing the relationship of RHR with coronary artery lesions and left ventriculai function,and investigating the association of RHR and left ventricular indexes with severity of coronary artery lesions by ordinal Logistic regression analysis.Results Coronary artery lesions differed significantly among the 4 groups (P < 0.05).the difference of LVDs、LVDd、LAs and EF was statistically significant (P < 0.05),but there was no difference of LAd (P > 0.05).Smoking、diabetes、the index of EF and LVDs were found to correlate with the degree of coronary lesion by Logistic regression analysis (P < 0.05).Conclusions RHR can affect the severity of coronary artery disease and left ventricular function in patients with coronary heart disease,and good control of RHR is beneficial for the prognosis of patients with coronary heart disease.Smoking、diabetes、the index of EF and LVDs were found to correlate with the degree of coronary lesion by Logistic regression analysis (P < 0.05).

ABSTRACT

Objective:To explore the positive expressions of biological markers human mammaglobin (hMAM) combined with matrix metallopeptidase 9 (MMP-9) and human epidermal growth factor receptor 2 (C-erbB2) mRNA in peripheral blood of the breast cancer patients with micrometastases,and to clarify its clinical application value in diagnosis of the micrometastases in peripheral blood of the breast cancer patients.Methods:A total of 74 patients with breast cancer,21 patients with breast fibroadenoma and 10 healthy controls were selected as the subjects.All the patients received surgical treatment and the peripheral blood was collected.The mRNA expression levels of hMAM,MMP-9 and C-erbB2 in peripheral blood were measured by the real-time fluorescent quantitative PCR.The positive expression rates of detection of hMAM,MMP-9 and C-erbB2 were compared,and the differences in detection of hMAM combined with MMP-9 and C-erbB2 between the patients with different clinicopathologic features were analyzed.Results:In the breast cancer patients with lymph node metastasis,the differences of positive expression rates of MMP-9 and C-erbB2 mRNA were significant (x2=6.450,P<0.05;x2=5.636,P<0.05),and the difference of positive expression rate of hMAM mRNA was sigificant between HER-2 positive and negative patients (x2=5.804,P<0.05).The positive expression rates of individual hMAM and combined with MMP-9 and C-erbB2 were 37.84% (28/74),59.46% (44/74) and 48.65% (36/74) in the breast cancer patients,the combined postive expression rate of these three kinds of markers was 64.86 % (48/74),which were higher than those in healthy controls group (x2=5.676,P<0.05;x2=3.102,P>0.05;x2=5.339,P<0.05;x2 =2.310,P>0.05),fibroadenoma of breast group (x2 =8.438,P<0.01;x2 =4.491,P< 0.05;x2 =7.982,P<0.01;x2 =4.844,P<0.05) and non-breast cancer group (healthy controls group+ breast fibroadenoma group) (x2 =13.093,P<0.01;xx2 =6.471,P<0.05;x2 =11.837,P<0.01;x2 =6.103,P< 0.05).The positive expression rates of individual hMAM and the joint detection in the breast cancer patients at stage Ⅲ + Ⅳ were higher than those in the patients at stage Ⅰ + Ⅱ;the positive expression rates of individual hMAM and combined with C-erbB2 were statistically significant (x2 =5.157,P<0.05;x2 =4.912,P<0.05).Conclusion:hMAM has a low positive rate in the diagnosis of micrometastases in the breast cancer patients,while hMAM combined with MMP-9 and C-erbB2 detection could improve the positive rates.which presents some clinical application value for the early diagnosis of breast cancer micrometastases.

ABSTRACT

Objective:To explore the positive expressions of biological markers human mammaglobin (hMAM) combined with matrix metallopeptidase 9 (MMP-9) and human epidermal growth factor receptor 2 (C-erbB2) mRNA in peripheral blood of the breast cancer patients with micrometastases,and to clarify its clinical application value in diagnosis of the micrometastases in peripheral blood of the breast cancer patients.Methods:A total of 74 patients with breast cancer,21 patients with breast fibroadenoma and 10 healthy controls were selected as the subjects.All the patients received surgical treatment and the peripheral blood was collected.The mRNA expression levels of hMAM,MMP-9 and C-erbB2 in peripheral blood were measured by the real-time fluorescent quantitative PCR.The positive expression rates of detection of hMAM,MMP-9 and C-erbB2 were compared,and the differences in detection of hMAM combined with MMP-9 and C-erbB2 between the patients with different clinicopathologic features were analyzed.Results:In the breast cancer patients with lymph node metastasis,the differences of positive expression rates of MMP-9 and C-erbB2 mRNA were significant (x2=6.450,P<0.05;x2=5.636,P<0.05),and the difference of positive expression rate of hMAM mRNA was sigificant between HER-2 positive and negative patients (x2=5.804,P<0.05).The positive expression rates of individual hMAM and combined with MMP-9 and C-erbB2 were 37.84% (28/74),59.46% (44/74) and 48.65% (36/74) in the breast cancer patients,the combined postive expression rate of these three kinds of markers was 64.86 % (48/74),which were higher than those in healthy controls group (x2=5.676,P<0.05;x2=3.102,P>0.05;x2=5.339,P<0.05;x2 =2.310,P>0.05),fibroadenoma of breast group (x2 =8.438,P<0.01;x2 =4.491,P< 0.05;x2 =7.982,P<0.01;x2 =4.844,P<0.05) and non-breast cancer group (healthy controls group+ breast fibroadenoma group) (x2 =13.093,P<0.01;xx2 =6.471,P<0.05;x2 =11.837,P<0.01;x2 =6.103,P< 0.05).The positive expression rates of individual hMAM and the joint detection in the breast cancer patients at stage Ⅲ + Ⅳ were higher than those in the patients at stage Ⅰ + Ⅱ;the positive expression rates of individual hMAM and combined with C-erbB2 were statistically significant (x2 =5.157,P<0.05;x2 =4.912,P<0.05).Conclusion:hMAM has a low positive rate in the diagnosis of micrometastases in the breast cancer patients,while hMAM combined with MMP-9 and C-erbB2 detection could improve the positive rates.which presents some clinical application value for the early diagnosis of breast cancer micrometastases.

ABSTRACT

Objective To observe the changes in help T cell 1/2 cytokines and transcription factors in murine cytomegalovirus (MCMV) infected newborn mice and to determine the effectiveness of intervention with thymopentin.Methods One hundred and twenty newborn BALB/c mice were randomized into a blank group,model group and thymopentin group,with 40 mice in each group.Mice in the model and thymopentin group received an intraperitoneal injection of MCMV suspension within 4-6 hours after birth at the dosage of 20 μ L to and tissue culture infections dose of 10431 U/0.1 ml,establish a systemic infection model,and the same volume of normal saline was injected into mice in the control group.Mice in the thymopentin group also received thymopentin 0.3 mg/(kg · d).On Day 3,5,7,10 and 14,8 mice in each group were sacrificed and their splenic tissues were harvested.Interferon (IFN) γ and interleukin (IL)-10 expression in the supernatant of a splenic lymphocyte culture was assessed by enzyme-linked immunosorbent assay.T-bet/GATA-3 mRNA in splenic tissues were analyzed using reverse transcription-polymerase chain reaction.One way ANOVA,including LSD and Dunnett T3 methods,was used for statistical analysis.Results (1) IFN-γ expression in the model and thymopentin groups peaked on Day 3,and was higher than that in the control group [(280.73 ± 14.88),(286.03 ± 15.44) and (149.42±5.43) pg/ml,respectively,F=183.532,P=0.000].Expression in the thymopentin group decreased after Day 3,and increased by Day 7.At Day 14,the IFN-γ expression level in the thymopentin group was higher than that in the control and model groups [(252.33±8.33),(149.07±7.05) and (148.57±4.53) pg/ml,respectively,F=385.487,P=0.000].(2) IL-10 expression in the model group gradually increased.By Day 14,the expression became obviously higher than that in the control and thymopentin groups [(71.19± 1.50),(36.67 ± 2.55) and (40.01 ± 1.28) pg/ml,respectively,F=523.670,P=0.000].IL-10 expression in the thymopentin group increased after Day 3 and decreased by Day 7.On Day 14,the expression in the thymopentin group was lower than that in the model group,but higher than that in the control group.(3) T-bet mRNA expression was obviously increased in the model and thymopentin groups.On Day 3,the expression was higher than that in the control group (relative value of gray-scale:0.74±0.02,0.71 ±0.04 and 0.30±0.01,respectively,F=741.630,P=0.000).By Day 3,the expression in the thymopentin group decreased,and gradually recovered on Day 5,and on Day 14 it was higher than that in the control and model groups (relative value of gray-scale:0.45 ± 0.01,0.30±0.01 and 0.30±0.01,respectively,F=257.571,P=0.000).(4) GATA-3 mRNA expression in the model and thymopentin groups increased on Day 3,and was higher than that in the control group (relative value of gray scale:0.48±0.02,0.53±0.01 and 0.33±0.01,respectively,F=345.167,P=0.000).On Day 14,the expression in the model group was higher than that in the thymopentin group,which was higher than that in the control group (relative value of gray-scale:0.99 ± 0.02,0.55 ± 0.02 and 0.34 ± 0.01,respectively,F=1 767.505,P=0.000).Conclusions A Th1/Th2 shift may be induced in MCMV infected neonatal mice,which manifests as a state of predominant Th2 response.Thymopentin can ameliorate this situation.

ABSTRACT

Objective To explore the role of high mobility group box protein 1 ( HMGB1 ) and receptor for ad-vanced glycation endproducts( RAGE) in lupus nephritis( LN) . Methods The serum and urine levels of HMGB1 were detected by ELISA in SLE patients and normal controls, and intrarenal expressions of HMGB1 and RAGE were detected by immunohistochemistry in renal tissues of SLE patients and normal-appearing renal tissues. Results The serum and urine levels of HMGB1 were significantly higher in SLE patients compared to healthy controls( P<0. 01 ) , in patients with active disease compared to those with inactive disease ( P <0. 05 ) , serum levels of HMGB1 were found to be significantly higher in patients with renal involvement compared to those without renal in-volvement(P<0. 05). In addition, the levels of serum HMGB1 showed positive correlation with SLEDAI,urine pro-tein(24 h) and lencocyte count (P <0. 05). The expression of urine HMGB1 showed positive correlation with SLEDAI ( P<0. 05 ) . Intermediate-intensity staining of HMGB1 and RAGE was detected in renal tubules in normal-appearing renal tissues, however, both renal tubules and glomerular cells had the strong expression of HMGB1 and RAGE in SLE patients. The intrarenal production of HMGB1 and RAGE in SLE patients was obviously higher than that in normal-appearing renal tissues(P<0. 05), and the expression levels of HMGB1 in SLE IV and IV+V were higher than those in SLE II ( P<0. 05 ) . The expression levels of RAGE in SLE IV were higher than those in SLE II(P<0. 05). Conclusion HMGB1 and RAGE may play key roles in the pathogenesis of LN and may associate with the pathology category of LN.

ABSTRACT

Objective To observe the expression of ITF in colon of mice infected by mouse cytomegalovirus (MCMV) and the in-volvement of ganciclovir(GCV) .Methods Forty-eight BALB/c young mice were randomly divided into blank group ,virus group and GCV group ,each with 8 mice .Mice in virus group and GCV group received injection of 100 μL MCMV virus suspension (TCID50105 .31 /mL) ,and GCV group was given intraperitoneal injection of GCV once a day at the dose of 50 mg/kg from day 0 (24 hours after vaccination of virus ) ,for 14 days .At the same time the virus group and blank group were given the same dose of normal saline as controls .Murine cytomegalovirus loads in livers and colons were measured by PCR .The expression levels of ITF in mRNA in colon were detected by RT-PCR .Results After MCMV injection ,mice in virus group manifested aggressively apparent poor appetite ,less activity ,fur laxly ,unresponsiveness to stimuli ,growth retardation ,body weight not increased .All liver and colon tissue MCMV-DNA PCR electrophoresis of virus group had positive strip ,while the blank group did not .GCV group also showed less bright positive strip when compared with the virus group .Expression level of ITF mRNA was significant higher in GCV group than virus group ,there was statistically significant difference(P<0 .05) .Expression of ITF mRNA in virus group were higher than that in blank group ,there was statistical difference(P<0 .05) .Conclusion ITF is regarded as a fast reaction peptide in the course of mucosa impairments ,so ITF plays a protective role in delayed healing process after acute MCMV infection .

ABSTRACT

Objective To investigate the alteration of Toll-like receptor (TLR) 2 and 9,myeloid cell differentiation 88 (MyD8),interleukin-2 (IL 2),interferon-γ(IFN γ) and interleukin-10 (IL-10) expression in spleen lymphocytes of cytomegalovirus (CMV) infected newborn murine.Methods Sixty-four neonatal mice were randomly divided into control group and CMV infected group (32 mice in each).Mice in CMV infected group were injected intraperitoneally with 20 μl CMV suspension (tissue culture infections dose 50 of 104.31 U/0.1 ml) at 4 6 hours after birth to create the systematic CMV infection models.At 3,5,7 and 10 days old,eight mice randomly selected from each group were sacrificed.Polymerase chain reaction was applied to detect CMV-DNA in liver,spleen,heart,lung,kidney and salivary glands of 7 day old mice.Western blot was used to measure the protein expression of T1R 2,9 and MyD88 in spleen lymphocyte,and enzyme linked immunosorbent assay was used to test the levels of IL-2,IFN-γand IL-10 in supernatant of spleen lymphocyte from 3,5,7 and 10-dayold mice.Independent samples t test and Pearson correlation analysis were applied as the statistical methods.Results On the 7th day after CMV infection,the protein expression of TLR 2,9 and MyD88 in spleen lymphocyte in CMV infected group increased to the maximum level (1.06±0.09,1.19±0.03 and 0.89±0.07),which were higher than those in control group (0.47±0.04,0.71 ±0.02 and 0.43±0.03),the differences were statistically significant(t=21.69,50.40 and 42.06,P<0.01,respectively).In CMV infected group,expression of IL-2 decreased to the lowest level on the 7th day after CMV infection [(16.9±7.3) pg/ml],IFN γ reached its peak level on the 5th day [(242.9±8.4) pg/ml] and IL-10 increased since the 5th day [(62.4±11.1) pg/ml],there were statistically significant differences as compared with control group [(51.9± 10.8) pg/ml,(135.3±10.5) pg/ml and (36.3±5.1) pg/ml](t=15.74,19.00 and 9.60,P<0.05,respectively).There was a negative correlation between TLR2,TLR9,MyD88 protein expression and IL-2 level (r =0.978,0.960 and-0.979,all P<0.01,respectively) and a positive correlation between TLR2,TLR9,MyD88 protein and IL-10 level (r =0.954,0.914 and 0.892,P < 0.01,respectively).Conclusions In the early phase of CMV infection (within 7 days),immunity is activated through TLR/MyD88 signal pathway resulting anti-infection effect.However,after the 7th day,Th1/Th2 cellular immunity imbalance allows virus to replicate easily.

ABSTRACT

Objective To study the relationship between the OATP1B1 521T > C genetic polymorphism and essential hypertension.Methods 164 essential hypertension subjects and 159 normotensive subjects were detected by the TaqMan-MGB probe real-time fluorescence quantitative PCR,and the results were compared with those of DNA sequencing.Results The frequencies of T/C genotype and C allele of OATP1B1 521T > C gene of the essential hypertension subjects were obviously lower than those of the normotensive subjects(T/C genotype:0.16 vs 0.25,P <0.05 ;C allele:0.10 vs 0.17,P <0.05),The difference was significant.Binary logistic stepwise regression analysis was used for evaluatine the risk factors of essential hypertension,there was significant relationship between OATP1 B1 52IT > C gene polymorphism and essential hypertension.Conclusion The SLCO1 B1 521T > C variant was common in Chinese essential hypertension population,but the difference of frequency of SLCO1B1 52IT > Cmuton between the essential hypertension patients and the normotensive controls was of obviously statistical significance,which indicates that the SLCO1B1521T > C variant maybe associate with essential hypertension.
